Case Manager- SMI

Department: Higley Integrated Care
Location: Gilbert, AZ

The Case Manager, using discretion and independent judgement, is responsible for the delivery of quality patient care to participants via the coordination of a participantís adult recovery team in securing and managing general medical and behavioral health services that aid in the participantís recovery from mental illness and/or addiction.

  • Participates in the assessment, evaluation and planning of patient care.
  • Develops individualized service plans for participants.
  • Provides community and home based services to enhance and achieve treatment goals.
  • Develops, facilitates and coordinates meetings, staffingís, case conferences and other meetings as appropriate.
  • Provides referrals to outside providers and assists in securing a variety of social services, including, but not limited to, relapse prevention services, medical care, food stamps, general assistance.
  • Maintains a minimum number of documented client contact hours per week (22 hours per week).
  • Maintains appropriate documentation (charting) and documents accurately and timely in accordance with agency and funding source requirements.
  • Communicates with participant (or participantís family) to coordinate consent to care, releases and/or referrals for provider care which includes behavioral care, general medical care, community resources, educational, social, judicial and other State agencies.
  • Coordinates transportation of participants to and from appointments when appropriate.
  • Consults with treatment staff to coordinate effectiveness of services contained in the participantís individual service plans and make adjustments where needed.
  • Provides outreach and follow-up on crisis contacts and missed appointments.
  • Meets with service providers to review utilization and monitor participant progress towards program goals.
  • Provides participant advocacy and education as needed, to ensure delivery of required services.
  • Develops transitional service plan (including discharge planning) with participants, prior to reentry into the community.
  • Performs other related duties in accordance with agency growth and changes.
  • Preserves the human dignity and the confidentiality of each patient.

Competencies Required:

  • Demonstrates quality written documentation that reflects correct spelling, grammar, appropriate use of behavioral health acronyms and appropriate use of therapeutic terminology.
  • Demonstrates team work and shared leadership thru positive and supportive communications.
  • Excellent technical capacity including detail orientation.
  • Collaborates (consults) with his/her supervisor or clinical consultant when support is needed.
  • Takes and follows direction well; attends supervision for one hour per week.
  • Flexible with excellent time management.
  • Maintain regular and reliable attendance and be punctual at all times for scheduled events.
  • Adheres to all administrative tasks as outlined in agency policies and procedures.
  • Knowledge of the standard principles, methods, practices, techniques, tools, and equipment used.
  • EMR record keeping of required and billable services.
  • Ability to organize, plan and communicate effectively and professionally.
  • Exhibit professional ethics and boundaries.
  • Must be able to work in a diverse, multi-cultural environment and be sensitive to the service populationís cultural and socioeconomic characteristics.
  • The ability to interact effectively with other team participants in a team based approach to care management.
  • All clinical staff may be required to carry the company on-call cell phone. This duty includes overnight, weekends and holidays.
  • Physical Requirements
  • Clinic/Office environment with frequent sitting, standing, writing and computer usage.
  • Must be able to climb stairs, with frequent standing and moving throughout facility or in the alternative, perform these physical requirements with or without reasonable accommodations.
  • Frequent to moderate physical effort (lift/carry up to 25 lbs.).
  • Frequent to moderate reaching, bending, stooping, kneeling or crouching.
  • Must be able to physically demonstrate competency in the operation of an agency transport vehicle.
  • Must be able to physically demonstrate competency in the operation of oneís own vehicle (if job includes driving clients).
  • Must be able to physically demonstrate the requirements of the job, as applicable.

Minimum Requirements:

Education/Experience:

  • GED/High school diploma with a minimum of 4 years of behavioral health experience or,
  • An associateís degree in the field of behavioral, or
  • A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
  • A bachelor's degree in the field of behavioral, or
  • A master's degree or higher in the field of behavioral health
  • Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, Credible preferred.
  • Valid AZ Driver License with clean driving record minimum of 5 years.
  • Interpersonal skills which allow for professional and positive relationships with co-workers, participants, families, and other community participants.
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.
  • Basic understanding of the DSM-V and ICD-10 preferred
  • Must pass background check and other pre-employment screening.
  • Must not be using any tobacco products
